Programs and Services
As a member of Abri, you receive all services covered by Medicaid from your doctors and other health care providers. You will also receive additional services.
Health Promotion
For all Abri Members, we offer Health Promotion services that provide you with information on preventative care and ways you can maintain your health.
Services provided include:
- Mailings of general health information on a variety of topics throughout the year
- A Member Newsletter produced at least twice a year with information about upcoming events, your benefits, ways to stay healthy and other important information
- Reminder cards or phone calls when it is time for things like check ups, immunizations, lead screenings, pap tests or mammograms
- Information about other things we can help you with such as help to quit smoking
First Two Years of Life Program:
- For Abri Members from birth to two years old, we offer a special Health Promotion program designed to help your child get off to a good, healthy start.
- Services provided include:
- Mailings set intervals during those years when your child needs preventive care
- Help getting your baby established with a Primary Care Provider (PCP)
- Special reminder calls when it is time for lead screenings
- Educational material about how your baby grows and stages of development
- Your child's complete immunization record mailed to you
- Help with making an appointment with your baby's PCP for preventive care
- Transportation to medical appointments (not available in all Counties, call us for details)

Condition Management
For our special health care needs Members, Abri offers Health Coaches. Health Coaches work with you in person or on the phone and offer disease management and support designed to inform, educate and promote self care for conditions such as diabetes, asthma, heart disease, high blood pressure, or certain others. Health Coaching services are offered to Members who need it based on their health status or who are referred to the program by their providers.
Services provided include:
- Mailings of condition specific information
- An assessment of your particular situation and health care needs
- Personalized information and coaching about your condition and ways in which to take care of yourself
- Phone call reminders for the things you need to do or see your provider about in order to take care of yourself
- Home visits when appropriate
- Help in getting medical services that are appropriate for your health care needs
- Help using skills you have and in developing new skills to take responsibility of managing your own condition
Prenatal Care Management
For our pregnant Members, Abri offers Prenatal Care Management by Health Coaches to help keep you and your unborn child healthy. Prenatal Care Management services are offered to all pregnant Members with special emphasis on Members with a high risk pregnancy.
Services provided include:
- Mailings of information related to pregnancy and childbirth
- An assessment of your particular situation and health care needs
- Help getting established with an OB doctor or midwife
- Referrals to community PNCC programs
- Help in setting up your baby with a Pediatrician
- Personalized information on pregnancy and the birth process and help with your questions
- Information on other community and neighborhood programs available such as WIC and the First Breath Program for help to quit smoking
Care Management
All Abri SSI Members are offered additional services from specialized Care Managers.
Services provided include:
- Conducting a health, social and functional assessment of your personal and social needs
- Development of a personalized Care Plan with you and your providers to help you reach your goals
- Help to determine short and long-term goals that meet the your health, social and functional needs
- Help navigating through the health care system
- Information on social services that may help with non-medical needs
- Provide basic information and education about Abri and your benefits
- Help in selecting a Primary Care Provider
Advocacy
All Abri members have access to a Member Advocate. Abri has Advocates for our BadgerCare Plus members and for our SSI members. Advocates for Abri SSI members have special training and experience in working with people with disabilities.
Services provided include:
- Help understanding your rights and benefits in working through the health care system
- Help in understanding healthcare benefits and how to get the most out of them
- Investigating complaints about the quality of care received
- Help filing grievances and appeals either at the Abri level or with the State
- Help in resolving problems with your health care
