How do I find a Primary Care Provider?
The back of your Member Handbook has a listing of Primary Care Providers for you to choose from. You can also call Customer Service at 888.999.2404 or 414.847.1776 and someone will help you.
Why do I need a referral to see a specialist?
Because your Primary Care Provider (PCP) knows your medical history the best. We believe that your PCP will know what type of specialist will be able to help you in your personal situation.
Do I get a separate card for Abri?
No, the ForwardHealth card you received in the mail is the card you will use.
Who do I call to get a replacement card?
You would contact Wisconsin Member Services at 1.800.362.3002.
Where do I go to apply for insurance?
You can apply over the internet or call Wisconsin Member Services at 1.800.362.3002.
My insurance with Abri Health Plan was terminated, how do I get reinstated?
You will need to call Abri’s Member Advocate at 414.847.1776 or call Wisconsin Member Services at 1.800.362.3002.
How do I find out why my insurance was terminated?
You will need to contact the Enrollment Specialist for that information at 1.800.291.2002.
Does Abri cover Flu Shots?
Yes.
Where do I go to get my eyes checked or go for eye glasses as an Abri member?
You can go to any Herslof Optician. You can contact the general scheduling office at 414.760.7400 or 414.462.3101.
I am not able to receive my medication at the pharmacy?
You will need to contact Wisconsin Member Services at 1.800.362.3002 about any prescription questions.
What are your regular business hours?
Customer Service is here to help you Monday - Friday 8:00 AM to 5:00 PM. We also have an After Hours Emergency line that is available to help you 24 hours a day, 7 days a week.

Depending on the location, you will need to contact the department of Social Services for the county you live in. The numbers you can call are:
